1. FINAL DOT CONSORTIUM / PROGRAM ENROLLMENT

Owner-Operator Essential $149 / year Annual random consortium enrollment and program administration. Testing is billed separately unless expressly included. Owner-Operator Complete $349 / year Includes Clearinghouse administration, annual query administration, one random DOT urine test with MRO review, and one QLD random DOT breath alcohol test. Additional selections and tests are billed separately. Employer: 2–10 drivers $199 / company / year Company-level annual enrollment. Testing and optional compliance services are billed separately unless expressly included. Employer: 11–25 drivers $299 / company / year Company-level annual enrollment. Testing and optional compliance services are billed separately unless expressly included. Employer: 26–50 drivers $399 / company / year Company-level annual enrollment. Testing and optional compliance services are billed separately unless expressly included. Employer: 51+ drivers Custom quote QLD determines pricing based on fleet size and administrative scope. The employer maintains its FMCSA Clearinghouse account and applicable query-plan or credit responsibility. Supervisor Reasonable-Suspicion Training $50 / supervisor Standalone training option. DOT Policy Assistance $195 Standalone written-policy assistance for employers not receiving it through another expressly defined package. MIS Report $150 / report Standalone report preparation. Post-SAP Follow-Up Program Management $295 / year Administrative management only. Testing is billed separately unless expressly included.
